Age UK-Cheshire East and Peaks & Plains Housing Trust, have come together to create a brand new service called Life links.
This free service is aimed at people of all ages across Cheshire East, who may be on the brink of physical dependence, or social isolation.
The service will ensure that they receive the support and advice they need to stay well.
